{"id":5268,"date":"2024-12-11T17:29:08","date_gmt":"2024-12-11T16:29:08","guid":{"rendered":"https:\/\/irccs.com\/patologie\/sarcomas-and-rare-tumors\/"},"modified":"2026-07-17T16:16:27","modified_gmt":"2026-07-17T14:16:27","slug":"sarcomas-and-rare-tumors","status":"publish","type":"pathology","link":"https:\/\/inoc.it\/en\/pathology\/sarcomas-and-rare-tumors\/","title":{"rendered":"Sarcomas and Rare Tumors"},"content":{"rendered":"\n<h2 class=\"wp-block-heading\"><span  id=\"pathology\" class=\"h2_anchor\"><\/span>Pathology<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Soft tissue sarcomas<\/strong> are rare tumors arising from the malignant transformation of cells within the so called <strong>soft tissues<\/strong>, including <strong>muscle, connective tissue, blood and lymphatic vessels, nerves, ligaments, and adipose tissue<\/strong>. More than <strong>50 histological subtypes<\/strong> have been identified, each with distinct biological and clinical features.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These tumors can develop in virtually any anatomical site. In more than <strong>50% of cases<\/strong>, they occur in the <strong>limbs or superficial trunk<\/strong>, while the remainder may arise in the <strong>head and neck, internal organs (such as the uterus), or the retroperitoneum<\/strong>, a deep abdominal region rich in fat and connective tissue.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some sarcoma subtypes are more typical of <strong>pediatric age<\/strong>, such as <strong>rhabdomyosarcoma<\/strong>, which affects approximately <strong>4\u20137 children per million under 15 years of age annually<\/strong>. Overall, sarcomas show three incidence peaks: <strong>childhood, young adulthood (18\u201330 years), and over 65 years<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">The Numbers in Italy<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Soft tissue sarcomas are <strong>rare diseases<\/strong>, with an incidence of approximately <strong>5 cases per 100,000 people per year<\/strong>, representing about <strong>1% of all adult cancers<\/strong>. According to the <strong>Italian Association of Cancer Registries <\/strong><a href=\"https:\/\/www.registri-tumori.it\/cms\/pubblicazioni\/i-numeri-del-cancro-italia-2023\">(AIRTUM)<\/a>, approximately <strong>2,230 new diagnoses<\/strong> of sarcomas and rare cancers were recorded in Italy in 2024, including <strong>1,289 in men and 941 in women<\/strong>.<br><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"symptoms\" class=\"h2_anchor\"><\/span>Symptoms<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">When sarcomas affect the limbs, they typically present as a <strong>mass or swelling that progressively increases in size over weeks or months<\/strong>. They are usually painless, except when in close contact with nerves or sensitive structures.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When located in the <strong>abdomen or retroperitoneum<\/strong>, symptoms are often non-specific and may include <strong>weight loss, reduced appetite, fatigue, or abdominal swelling<\/strong>. In some cases, the tumor may cause <strong>intestinal obstruction, gastrointestinal bleeding, or lower limb edema due to vascular compression<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The lack of specific symptoms often leads to <strong>delayed diagnosis<\/strong> in abdominal sarcomas compared with limb localized disease.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"risk-factors\" class=\"h2_anchor\"><\/span>Risk Factors<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Given the heterogeneity of soft tissue sarcomas, there are no universal risk factors. However, several conditions may increase risk:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Radiation exposure<\/strong>, particularly previous <a href=\"https:\/\/inoc.it\/en\/medical-area\/radiotherapy\/\">radiotherapy treatments<\/a><\/li>\n\n\n\n<li><strong>Chemical exposure<\/strong>, including vinyl chloride, dioxin, and certain pesticides<\/li>\n\n\n\n<li><strong>Viral agents<\/strong>, such as HBV and HHV8 (associated with Kaposi sarcoma)<\/li>\n\n\n\n<li><strong>Genetic predisposition or familial syndromes<\/strong>, although rare<\/li>\n\n\n\n<li><strong>Lymphedema<\/strong>, often following lymph node dissection, associated with lymphatic sarcomas<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"sarcomas-and-rare-tumors-program\" class=\"h2_anchor\"><\/span>Sarcomas and Rare Tumors Program<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A dedicated <strong>multidisciplinary program<\/strong> is active at INOC &#8211; Istituto Nazionale Oncologico Candiolo, ensuring timely diagnosis, individualized treatment, and continuous follow up.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Institute actively participates in <strong>clinical and preclinical research<\/strong>, with the aim of improving diagnostic and therapeutic strategies and reducing the burden of so called <strong>\u201corphan diseases\u201d<\/strong>, for which evidence and specific treatments are often limited due to low incidence.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Management of rare tumors requires strong <strong>national and international collaboration<\/strong>. The Institute serves as a regional and national reference center and actively participates in networks such as the <strong>Italian Sarcoma Group (ISG)<\/strong>, the <strong>Rare Tumor Network (RTR)<\/strong>, <strong>EORTC<\/strong>, and <strong>ERN EURACAN<\/strong>. Within these networks, the Institute participates in and coordinates clinical and translational studies aimed at improving treatment strategies.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Pathologies Treated<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The program covers a broad spectrum of rare tumors, including:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Soft tissue sarcomas:<\/strong> liposarcoma, leiomyosarcoma, undifferentiated pleomorphic sarcoma, synovial sarcoma, angiosarcoma, solitary fibrous tumor, malignant peripheral nerve sheath tumor, rhabdomyosarcoma, alveolar soft part sarcoma, fibrosarcoma, desmoplastic small round cell tumor, hemangioendothelioma, PEComa, and other histological types<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Gastrointestinal stromal tumors (GISTs)<\/strong><\/li>\n\n\n\n<li><strong>Bone sarcomas:<\/strong> osteosarcoma, Ewing sarcoma, chondrosarcoma, giant cell tumor, and other rare forms<\/li>\n\n\n\n<li><strong>Kaposi sarcoma<\/strong><\/li>\n\n\n\n<li><strong>Desmoid tumor \/ aggressive fibromatosis<\/strong><\/li>\n\n\n\n<li><strong>Tenosynovial giant cell tumor (pigmented villonodular synovitis)<\/strong><\/li>\n\n\n\n<li><strong>Merkel cell carcinoma<\/strong><\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Multidisciplinary Approach<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The program is based on a <strong>multidisciplinary<\/strong><strong> approach<\/strong> involving <strong>medical oncologists, surgical oncologists, orthopedic surgeons, ENT surgeons, gynecologic oncologists, urologists, radiation oncologists, pathologists, radiologists, nuclear medicine specialists, gastroenterologists, and pain specialists<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dedicated <strong>psycho-oncologists, dietitians, physiotherapists, social workers, and nurses<\/strong> support the patient throughout the care pathway, ensuring comprehensive and continuous care.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"diagnosis-and-examination\" class=\"h2_anchor\"><\/span>Diagnosis and Examination<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The <strong>diagnostic pathway<\/strong> for sarcomas and rare cancers generally begins when the primary care physician or other specialists identify <strong>suspicious symptoms or findings,<\/strong> when a radiologic examination suggests the presence of a lesion, or after the detection of a histologic diagnosis of a rare neoplasm belonging to this category.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Patients who enter the <strong>Reception and Service Center (CAS)<\/strong> are evaluated based on their clinical condition and the assessments already performed. During this phase, all necessary tests are indicated to complete the diagnostic process, precisely define the staging of the disease, and plan the most appropriate course of treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Common diagnostic and staging tests in soft tissue\/bone sarcomas and rare tumors include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Magnetic resonance imaging (MRI)<\/strong> of the affected site<\/li>\n\n\n\n<li><strong>Computed tomography (CT)<\/strong> chest-abdomen with and without contrast medium and of the involved body district<\/li>\n\n\n\n<li><strong>Total-body PET<\/strong>, when indicated<\/li>\n\n\n\n<li><strong>Bone scintigraphy<\/strong>, if necessary<\/li>\n\n\n\n<li>For <strong>gastrointestinal stromal sarcomas (GISTs)<\/strong>, an endoscopic (gastroscopy or colonoscopy) or echendoscopic procedure with biopsy collection may be useful<\/li>\n\n\n\n<li><strong>Ultrasound-guided biopsy or CT scan<\/strong><\/li>\n\n\n\n<li><strong>Surgical bio<\/strong>psy, in case of failure of guided biopsy<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Tumor Characterization<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Definitive diagnosis is based on <strong>histological examination<\/strong> of biopsy samples performed by the pathologist. Accurate diagnosis is essential for treatment selection.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Due to the rarity and complexity of sarcomas, pathological evaluation requires specialized expertise. At INOC &#8211; Istituto Nazionale Oncologico Candiolo, integrated <strong>immunohistochemistry, flow cytometry, cytogenetics, and molecular biology techniques<\/strong> are used to support a precision medicine approach.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span  id=\"therapies\" class=\"h2_anchor\"><\/span>Therapies<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">After diagnosis, the<a href=\"https:\/\/inoc.it\/en\/pathology\/sarcomas-and-rare-tumors\/#multi-disciplinary-team\"> <strong>GIC multidisciplinary team<\/strong><\/a> evaluates tumor type, size, extent of disease, patient age, and clinical condition to define an individualized treatment plan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The therapeutic strategy is discussed with the patient, including equivalent alternatives when available. Depending on disease stage, treatment may include <strong>surgery, chemotherapy, targeted therapy, immunotherapy, radiotherapy, or combined approaches<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Whenever possible, patients are offered participation in <strong>clinical trials<\/strong> to access the most advanced treatment options. The Sarcomas and Rare Tumors Program includes multiple research protocols evaluating chemotherapy, targeted therapies, and immunotherapy.<\/p>\n\n\n\t<div id=\"block_6a5a393a8a6d7\" class=\"accordion-block acf-block\">\n\t\t<div class=\"my-4 lg:my-6\">\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Surgery<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><b>Surgery remains the main treatment for sarcomas<\/b><span style=\"font-weight: 400;\">, often in combination with other therapeutic modalities. High complexity procedures require <\/span><b>multidisciplinary planning<\/b><span style=\"font-weight: 400;\">, as tumor site, anatomical location, and histopathological features vary significantly between cases and involve multiple specialist disciplines.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Unfortunately, some patients initially receive <\/span><b>inadequate surgical treatment<\/b><span style=\"font-weight: 400;\">, which may negatively affect both <\/span><b>disease outcome and quality of life<\/b><span style=\"font-weight: 400;\">. For this reason, it is essential that surgery is performed in <\/span><b>highly specialized centers<\/b><span style=\"font-weight: 400;\"> or in facilities with the required expertise and resources.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Surgical treatment must always be <\/span><b>radical and complete<\/b><span style=\"font-weight: 400;\">, ensuring full removal of all cancer cells, as <\/span><b>radiotherapy cannot compensate for incomplete surgical excision<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In selected cases, <\/span><b>metastasectomy<\/b><span style=\"font-weight: 400;\">, such as resection of <\/span><b>lung metastases<\/b><span style=\"font-weight: 400;\">, may be considered when a <\/span><b>complete macroscopic and functional disease clearance<\/b><span style=\"font-weight: 400;\"> is achievable.<\/span><\/p>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Postoperative Recovery &#8211; ERAS Protocol<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><span style=\"font-weight: 400;\">At INOC &#8211; Istituto Nazionale Oncologico Candiolo, surgical care is supported by the <\/span><a href=\"https:\/\/inoc.it\/en\/medical-area\/anesthesia-and-resuscitation\/\"><b>Enhanced Recovery After Surgery (ERAS) protocol<\/b><\/a><span style=\"font-weight: 400;\">, which aims to promote a rapid return to independence, reduce hospital stay, and decrease postoperative complications.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The protocol is based on a <\/span><b>multidisciplinary approach<\/b><span style=\"font-weight: 400;\"> involving surgeons, anesthesiologists, dietitians, nurses, psychologists, physiotherapists, and social and healthcare workers, all coordinating the patient\u2019s perioperative pathway.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Key elements of the program include:<\/span><\/p>\n<ul>\n<li aria-level=\"1\"><b>Optimal pain control<\/b><\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\"><b>Minimally invasive surgical techniques<\/b><\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\"><b>Preoperative counseling<\/b><\/li>\n<\/ul>\n<ul>\n<li aria-level=\"1\"><b>Early rehabilitation<\/b><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Additional components of the protocol include:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Nutritional support<\/b><span style=\"font-weight: 400;\">, including avoidance of prolonged preoperative fasting<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Personalized anesthesia<\/b><span style=\"font-weight: 400;\">, enabling early return to normal oral intake<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Restricted use of drains, catheters, and intravenous lines<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Early mobilization<\/b><\/li>\n<\/ul>\n\n\t\t\t\t<\/div>\n\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t<div class=\"accordion group border-b border-white\/5\">\n\t\t\t<div class=\"flex items-center cursor-pointer group-hover:text-blue-light group-[.accordion--open]:text-red transition-colors\">\n\t\t\t\t<div class=\"rotate-90 text-blue-lighter group-[.accordion--open]:rotate-180 group-[.accordion--open]:text-red transition-all\">\n\t\t\t\t\t<svg width=\"18\" viewbox=\"0 0 500 500\" fill=\"currentColor\">\n\t\t\t\t\t\t<polygon points=\"250,60 100,400 400,400\"\/>\n\t\t\t\t\t<\/svg>\n\t\t\t\t<\/div>\n\t\t\t\t<h3 class=\"font-bold ml-5\">Chemotherapy<\/h3>\n\t\t\t<\/div>\n\t\t\t<div class=\"accordion-content max-h-0 overflow-hidden transition-all\">\n\t\t\t\t<div class=\"pb-5 pl-[38px] pt-1\">\n\t\t\t\t\t<p><b>Chemotherapy<\/b><span style=\"font-weight: 400;\"> uses drugs capable of destroying cancer cells by targeting their high rate of proliferation compared to healthy cells. However, this mechanism can also affect normal tissues, leading to <\/span><b>side effects<\/b><span style=\"font-weight: 400;\">, which are generally reversible after treatment discontinuation. Before starting therapy, the oncologist discusses the planned drugs with the patient and strategies to mitigate their impact.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b>When it is Used<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">In <\/span><b>soft tissue sarcomas<\/b><span style=\"font-weight: 400;\">, <\/span><b>bone tumors<\/b><span style=\"font-weight: 400;\">, and other rare cancers, chemotherapy may be used at different stages of treatment:<\/span><\/p>\n<p><b>After surgery (adjuvant)<\/b><span style=\"font-weight: 400;\">: to reduce the risk of recurrence<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><b>Before surgery (neoadjuvant)<\/b><span style=\"font-weight: 400;\">: to reduce tumor size and facilitate surgical removal; in selected cases it may be combined with radiotherapy after multidisciplinary evaluation<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><b>Metastatic disease<\/b><span style=\"font-weight: 400;\">: to slow disease progression, reduce symptoms, and improve quality of life<\/span><\/p>\n<p>&nbsp;<\/p>\n<h3><b>Mode of Administration<\/b><\/h3>\n<p><b>Intravenous<\/b><span style=\"font-weight: 400;\">: the most common route, administered in day hospital or inpatient settings; infusion duration may range from minutes to several hours<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><b>Oral<\/b><span style=\"font-weight: 400;\">: in tablet form, as in some cases of <\/span><b>gastrointestinal stromal tumors (GISTs)<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Treatment is given in <\/span><b>cycles<\/b><span style=\"font-weight: 400;\">, alternating periods of therapy with rest periods.
